Who should attend: Physicians and other health care professionals seeking up-to-date information about addiction issues affecting their patients. Practitioners interested in networking with other regional addiction professionals.

Educational objectives:

Learn the strategy and results of Maine Governor’s Exec.Order #2 dealing with the State’s Opioid Response
Learn two advocacy issues for NNESAM and two for ASAM
Learn about addiction in the Jewish community, the value of Jewish texts in recovery
a. Learn about in-office testing of reaction time; its usefulness in Medication Assisted Treatment
b. Discuss indications and strategies for tapering buprenorphine.
c. Discuss conflicting frameworks for thinking about “problem patients”.
Learn evidence / best practices for harm reduction. Learn opportunities to access same in northern New England
Learn the epidemiology of infection due to injection drug use and Maine Med Ctr’s experience with endocarditis
Learn strategies for managing thorny clinical problems faced by clinicians who are providing office-based buprenorphine treatment for outpatients in community settings.

About the speakers

James Berry, MD, FASAM

After practicing Family Medicine in rural Maine for 30 years, Dr. Berry worked full time in addiction medicine at Mercy Recovery Center in Portland from 2009 until it closed in 2015. In semi-retirement, he works in addiction and pain management at one of Mercy’s outpatient sites and at a local mental health agency, SMART Child and Family Services. He is board certified in Family Medicine and Addiction Medicine.

Mark Bucksbaum, MD

Dr. Bucksbaum is a Biomedical Engineer from Case Western Reserve University (1984) and received his medical degree from St. Georges University School of Medicine in 1984. Dr. Bucksbaum is board certified in Physical Medicine and Rehabilitation Pain Management and as an Independent Medical Examiner. Dr. Bucksbaum is the author of the best practice model for chronic pain call the CLIPP program and is the former Medical Director for Pain Management at Maine General Medical Center. Dr. Bucksbaum currently practices Pain Management and Addiction Medicine at the Center for Integrative Medicine in Rutland Vermont.

Judy Burk, MD

Dr. Burk was born in Ohio and graduated from Univ. of Cincinnati College of Medicine. She came to Maine in 1983 as a GP in Lubec, Maine to fulfill her Service Corps obligation. She returned to training in Cincinnati, came back to Maine and became board certified in psychiatry. She has practiced psychiatry in many settings – both Maine state hospitals, Acadia hospital inpatient and outpatient, and at Maine’s juvenile corrections center in Charleston Maine. While working with incarcerated youth she first encountered the opiate epidemic. She pursued ASAM, ABAM and finally ABMS Preventive Medicine Addiction Certification. She has worked at Acadia’s methadone clinic, and provided addiction consultation and treatment, including medication assisted treatment, in general psychiatry settings. She has been an officer in NNESAM since it was incorporated in 2010.

R. Jeffrey Goldsmith, MD, DFASAM, DLFAPA

Dr. Goldsmith is the lead physician consultant for Maine for the Opioid Response Network (STR- TA), a SAMHSA grant awarded to the American Academy of Addiction Psychiatry (AAAP) with a coalition of 27 national professional organizations to provide technical assistance. Dr. Goldsmith is a past President of American Society of Addiction Medicine (ASAM) and is a Diplomate of the American Board of Preventive Medicine and Certified by the American Board of Psychiatry and Neurology. He harkens from Cincinnati where he completed research in addictions and provided clinical services.

Kerry Nolte, Ph.D, FNP-C

Dr. Nolte is an assistant professor of nursing at the University of New Hampshire and continues her clinical practice as a Family Nurse Practitioner at Families First, a Federally Qualified Community Health Center. In her clinical work, Kerry is particularly concerned with engagement in care and supporting people who use drugs. She received her MS in nursing from UNH and her PhD in nursing from Northeastern University. Her international experience includes serving on Project Hope in the South Pacific and leading global education programs for UNH in Ghana. Kerry’s research interests focus on communication and reducing harms associated with using drugs. Her previous inquiry has focused on HIV prevention in vulnerable population. Kerry serves the NHNPA working on policy and represents NHNPA on the Governor’s Commission on AOD Healthcare Task Force. Kerry is one of the authors of Have The Conversation: Caring for People Who Inject Drugs, A Guide for NH HealthCare Providers. She has been recognized for her work by the NH Nurses Assoc and the New Hampshire Magazine with an Excellence in Nursing Award

Rabbi Ellie Shemtov

Rabbi Shemtov was ordained by the Academy for Jewish Religion in Yonkers, NY in 2014 and most recently, since July of this year, has been serving as spiritual leader of the Rutland Jewish Center, Rutland, Vermont. She has degrees in Music and Library Science and has worked in several position in those fields. Prior to accepting the Rutland position this past summer, she was the Head of Film Cataloging for the Library of Congress. She has a particular interest in substance use disorders after experiencing the death of her ex-husband. Her rabbinical school thesis, “Beit Yosef: a Sacred Space for Healing from the Disease of Addiction,” investigated Beit T’Shuvah, a residential treatment center and full-service congregation in Los Angeles, as a model of what might be possible on the East Coast. She is committed to raising awareness of this disease in the Jewish community, and to teach how the Jewish texts can be used in recovery.

Gordon H. Smith, Esq. gordon.smith@maine.gov

Mr. Smith, in January 2019, was appointed to serve as the Director of Opioid Response for the State of Maine. The position will marshal the collective power of state government to combat the opioid epidemic. Prior to this appointment, Smith was the Executive Vice President of the Maine Medical Association. A Maine native, he graduated from the University of Maine in 1973 and from the Boston College Law School in 1976. He practiced law privately before going to the Maine Medical Association as General Counsel in 1981. He is a past Chairman of the American Society of Medical Association Counsel, and the AMA/State Medical Society Litigation Center. Smith has also served as Chair of the Maine Health Data Organization and of the Executive Committee of the Advocacy Resource Center of the American Medical Association. Smith is also a former Chair of the Board of Quality Counts, a regional quality improvement collaborative and a former board member of the Maine Association of Area Agencies on Aging. He is a current board member of the Daniel Hanley Center for Health Leadership. Smith lives in East Winthrop.

Kinna Thakarar, D.O., MPH

Dr. Thakarar is an Infectious Disease (ID) physician with a special interest in addiction medicine, HIV/HCV and health services research. She received a BA in Health and Societies from University of Pennsylvania, her DO degree from Philadelphia College of Osteopathic Medicine, and an MPH from Johns Hopkins Bloomberg School of Public Health. She completed internal medicine residency at Cambridge Health Alliance and infectious disease fellowship at Boston Medical Center in 2015, where she was part of BU-CHART, an NIH-funded research training program in HIV/AIDS research for underserved populations, and also completed an NIH-funded fellows’ immersion training program in addiction medicine. She is currently an attending physician at Maine Medical Partners, Portland, Maine and an Assistant Professor of Medicine at Maine Medical Center / Tufts University School of Medicine.
